TRAUMA CENTER CRITERIA

MECHANISM OF INJURY
  1. Falls of 2 stories (15 feet) or more.

  2. Survivor of a motor vehicle accident in which there was a death of an occupant.

  3. Patient struck by a vehicle moving faster than 20 miles per hour.

  4. Patient ejected from a vehicle. Pediatric patients not secured in vehicle.

  5. Severe deformity of the vehicle.

  6. A vehicle rollover.
PHYSICAL FINDINGS
  1. Decreased capillary refill.

  2. Pulse rate less than 50 or greater than 120 beats per minute for an adult.

  3. Systolic blood pressure of 90 mm/Hg or less for an adult.

  4. Respiratory rate less than 12 or greater than 28 breaths per minute with inadequate volume for an adult.

  5. Glasgow Coma score of less than 13, or the presence of other significant CNS disturbance.

  6. Adult Revised Trauma score of 11 or less, or a Pediatric Trauma Score of 8 or less.

  7. All trauma to the head, neck, chest, abdomen, or pelvis.

  8. Pelvic fracture, or 2 or more proximal long bone fractures.

  9. Flail chest, pneumothorax, or pericardial tamponade.

  10. Minor burns with associated hemorrhage or major trauma.
EXTREMIS CRITERIA
  1. Traumatic cardiac arrest.

  2. Obstructed or unmanageable airway.

MAJOR TRAUMA PATIENTS SHOULD BE TRANSPORTED TO A TRAUMA CENTER PROVIDED IT IS NOT MORE THAN 20 MINUTES AWAY
